New Patient Registration

You are welcome to register with the practice if you live within the practice area.

The Area We Cover

Our main surgery is situated in Fowey, with two branch surgeries, at Par Health Centre and on Polruan Quay.

Our practice boundary stretches from Tregrehan on the west to Lanteglos on the east side of the River Fowey, and north as far as St. Blazey.

If you move out of our practice area you should register with a practice local to your new address.


We welcome new patients, residing in our catchment area, to register you need to either collect, or print and complete a New Patient (GMS1 form) and a New Patient Questionnaire (which are both downloadable via the links below) and return to the surgery.  If you have the following available, photographic ID (ie passport or driving licence) and evidence of your address that would be very helpful.

If you have your current medical card you should bring it with you. If you are unable to find it the reception staff will be able to provide alternative forms.

Practice Facilities

For the disabled

Fowey Surgery and Par Health Centre are easily accessible by people in wheelchairs. Polruan Surgery has quite a narrow entrance with one small step.

Other help

Hearing loops are fitted in Fowey and Par reception offices.

The Practice also has access to a translation/interpreter service.
